Community Impact Report

Strengthening our communities

Making our communities stronger and healthier is at the core of everything we do at Sanford Health. As the nation’s largest nonprofit rural healthcare system, we’re committed to delivering world-class care no matter where you live.

Each year, we reinvest resources into programs, services and initiatives that help strengthen community health and expand access to care.

This commitment extends beyond our medical centers and clinics. We’re active in the communities we serve and work alongside local caregivers and leaders to improve public health and address health disparities to help make our communities stronger and more resilient.

Affordable, high-quality healthcare across our regions

We consider it a privilege to provide world-class healthcare services to every patient.

Across the rural communities we serve, many individuals and families are uninsured, underinsured or face financial barriers to care. We are committed to ensuring our patients receive the care they need, regardless of their ability to pay.

By embracing virtual care innovations, developing a robust clinical workforce and leading groundbreaking medical research, Sanford is improving outcomes and ensuring access to high-quality healthcare in the communities we serve.

Subsidized health services

Sanford Health invests in programs and services that are vital to rural communities but often operate at a financial loss. Investing in these programs helps ensure patients can continue to access needed, specialized care close to home.

Behavioral health

We’re expanding access to mental healthcare through inpatient services, outpatient programs and virtual appointments.

Specialty care

Sanford’s support helps ensure patients have access to vital specialty care services, including oncology, obstetrics and gynecology, and emergency care.

Emergency care

Maintaining timely, accessible emergency services across our communities ensures patients can receive critical care when they need it most.

Uncompensated Care

Sanford Health is committed to reducing financial barriers for thousands of patients. We believe that financial circumstances should not determine whether someone can access care.

Charity care program

Sanford Health proactively identifies patients who may qualify for assistance before a bill is sent, with 25% to 30% of recipients qualifying automatically.

Financial assistance

All patients receiving emergency or medically necessary care can apply for financial assistance. Each year, 95% of applicants receive financial support, and among those approved, 85% receive full assistance.

Care for uninsured or underinsured patients

Sanford partners with Elevate Patient Financial Solutions (Elevate PFS) to provide financial assistance for uninsured patients. Elevate PFS helps identify the most appropriate assistance using a variety of resources.

Medicaid and Medicare

Sanford Health helps bridge the gap between reimbursement rates and cost of care.

Sanford absorbed more than $302 million in Medicaid and Medicare underpayments in 2024.

Access for seniors and vulnerable populations

Many communities that Sanford Health serves have older populations and higher rates of chronic illness than national averages.

Sanford is committed to providing comprehensive care for Medicare and Medicaid beneficiaries who rely on our healthcare services.

Supporting rural hospitals and clinics

Delivering care across wide geographic areas requires investments in emergency services, specialty care and rural facility staffing.

Sanford helps absorb these costs, which helps maintain quality, timely access to care throughout all our communities.

Community health improvement services

Sanford Health supports prevention programs, population health initiatives and community-based services that help people stay healthy before they need acute care.

Community Health Needs Assessment

The Community Health Needs Assessment helps us understand significant unmet health needs and prioritize local partnerships and initiatives to address them.

While individual choices are important, health also depends on several community factors like food security, public safety, education and air quality.

Early intervention

Rural communities face transportation barriers and higher rates of chronic disease. We’re extending healthcare beyond clinical settings by investing in prevention and early intervention.

For example, Sanford has invested in virtual care to bring specialists to rural, underserved areas and virtual behavioral health appointments to increase access to mental health care.

Community building activities

Sanford Health focuses on programs that help improve the everyday factors that affect patients’ health, including education, employment and community resources.

Creating access to stable employment

We support leader and career development opportunities that in turn support patients, families and our communities.

Partnering with strong local institutions

Sanford collaborates with local leaders, including chambers of commerce, to strengthen communities and address local needs to support long-term health.

Connecting to community resources

Sanford Helps offers patients a website to connect to community resources, such as financial assistance, food pantries, housing, medical care and other free or reduced-cost help.

Health professions education

Sanford Health supports healthcare learners in all stages, because building a strong future workforce helps improve access to care in our communities.

Student training

In 2024, Sanford supported more than 400 residents and fellows across a range of specialties, helping prepare the next generation of caregivers.

Aspire by Sanford

Aspire by Sanford promotes healthcare careers to students of all ages. The program offers hands-on experiences and exposure to healthcare career opportunities.

Building the healthcare workforce

Sanford Health leaders have connected with educational leaders, legislative members, community members, civic leaders and philanthropic partners to discuss educating and investing in the next generation of healthcare professionals.

Medical research

Sanford Health is committed to bringing access to medical research and clinical innovation to our rural populations.

Comprehensive clinical research network

In 2024, Sanford invested $19.7 million in medical research, which supported nearly 1,100 clinical trials that connect patients to cutting-edge treatments and evidence-based care.

Community-based research

Sanford Research is helping close the gap to early-phase clinical trials.

In many communities, access to early-phase clinical trials is limited and requires patients to travel long distances to major academic centers. At Sanford, most participants are treated close to home.

Innovation in rural healthcare

Sanford Health is invested in bringing innovative care closer to home, ensuring patients have access to new possibilities, even when options are limited.

For example, Sanford Research scientists are investigating new cancer treatment options through a robust clinical trial program – bringing a dynamic approach to cancer care.
